Toothache!! Hurts like ****

First off, I don't have a toothache, I'm a dentist.

I have been reviewing several of the older threads about toothaches and dental infections. And, YES! people have died from oral/tooth related infection.

The posts of advice vary from OK to **** poor.

My best advice, if you have a serious toothache and have any access to a dentist...GO! or call your dentist at any time 24/7.

If your dentist is unavailable in the evenings or on weekends, dump him and find one who is.

I have seen patients after hours or on weekends for dental emergencies MANY times in my 40 years of practice.

I consider it a privilege to take another human being out of pain. If your dentist doesn't have a similar philosophy, dump him and find one who does.

Now, what do you do when you have no access to a dentist?

WARNING! the following is for informational reference only. Not to be considered treatment advice:

1] You should have already prepped fish antibiotics. Specifically FishMox Forte. You should have a minimum of 60 capsules on hand. This is the same as Amoxicillin 500mg for human dosage. NOT for those who are allergic to penicillin.

2] If your toothache is severe (pain with swelling/redness in the face), take two capsules every 8 hours for the first two days (6 per day), then 1 capsule every eight hours for another eight days.

3] After the first two doses (4 capsules), you can use warm packs on your face in the area of the infection. This will make the swelling WORSE at first, but it will relieve the pressure from the immediate area around the tooth and the pain will decrease drastically. NEVER put cold packs on your face to treat infection/pain.

4] Pain control may still be difficult. If you are fortunate, Ibuprofen (600mg) will be adequate, But often Ora-gel and/or oil of cloves will be necessary until the amoxicillin kicks in.

5] If the pain is from an abscessed tooth (as opposed to gum infection), this treatment will only give temporary relief, and the infection will return if the tooth is not treated by root canal or extraction.

6] Poking the swollen area around a tooth can allow infection to drain, but there is a significant risk that the infection can be driven deeper into the area or even into the blood stream and you can get seriously dead from that infection.

CHLORHEXIDINE GLUCONATE ORAL RINSE, 0.12% AKA Peridex is a useful short term treatment for mild to moderate gum infection.

It is almost useless for treatment of severe tooth abscess that needs a root canal or extraction.

It is NOT recommended to use this rinse long term since it will result in very bad staining of the teeth that is very tough to resolve.

I'm easy to find, with little imagination, in Denver listings.

Lord Opie and DocHudson, you are both welcome to relocate to the Kingman, AZ area. We have cheap land nearby with good wells, year round growing of veggies and plenty of sunshine for solar. I speak as one who lived in Colorado (mostly the Denver area) for 30 years. We have excellent medical facilities (affiliated with the Scottsdale, AZ., Mayo Clinic) if you need such. We are people friendly and gun friendly and Prepper friendly.

Doc, is here any truth that the nerves in teeth tend to retract as one ages? I had some bad teeth that the dentist showed me and I find it hard to believe that they caused me no pain. The sharp edges were driving me nuts though. I had let it go until it started disintegrating. He didn't act surprised that it caused me no discomfort. Mentioned nerve retraction but didn't go into any detail.

BTW. I will never eat anything that has been shot with a shotgun again. Chomped down on a #4 shot and cracked a back molar years back. I never get toothaches. Just wondering.

Scruggs, yes, healthy teeth continuously lay down new tooth material (dentin) from the inside.

This process results in the nerve chamber shrinking and the nerve gets further away from the outer surface slowly over many years.

The tooth will therefore be much less sensitive as we age. Try to remember how sensitive our new teeth were to cold things when we were kids.

In some cases, my patients who are big time night time grinders, the nerve will withdraw (by laying down LOTS of new dentin) to the point that the nerve chamber is either microscopically small or gone altogether.

I have seen quite a few patients who have ground/broken their teeth down to just above the gum line, and the nerve is still snug behind late grown dentin. There is almost never any pain or sensitivity in these cases.

OTOH, I have seen exactly the opposite. There are many more people (usually ladies) who grind their teeth only slightly and they have lots of pain and sensitivity in the teeth, jaws and even headaches.

One of THE WORST Old Wives tales is to put aspirin on an aching tooth or on the gums.

Aspirin is a FREAKING ACID!!!! and will burn the living **** out of your gums and cheek.

Use Ora-jel, oil of cloves, dilute tea tree or oil of oregano....but NEVER, EVER!! place aspirin on the gums or tooth

For further reference:

Oil of Cloves is also known as Eugenol. Both are available at Amazon:

Mix either Oil of cloves or Eugenol with zinc oxide powder to make a thick paste which is very useful for temporary fillings or re-cementing crowns that come off.

As a kit:

There is a kit sold otc at drug stores that is a very compact version.

I carry one at all times,and I often travel with my hunting partner,a dentist that has been a friend for over 40 years.

Best way to treat a tooth ache = or any pain,have a SDr in the camp !!!.

Doc, Thanks so much for this. I have had the worst experiences with dentists. You wouldn't happen to have any colleagues in Virginia, would you? You are a tad too far and I doubt my truck would make it to you! ...how does one approach a dentist about survival issues? You have given us a lot of info on here and I am grateful for it. Thanks again.

skidmarkjones; Sadly, no I don't have a classmate/colleague that I could refer you to.

I suggest that you ask around at work, neighborhood, church, friends who love their dentist and why. Best case basis, you are looking for the same name from several sources.

If all else fails, find your closest dentists and look them up on yelp or other feed back sources.

Now, about survival issues...that's a tougher one.

I generally only discuss these issues with my closest friends/patients. You might ask about the possibility of obtaining antibiotics in large bulk (eg: 500 capsules of amoxicillin) for long term emergency storage.

Another way to broach the subject is to determine if your dentist is a gun owner/2A supporter.

If you determine that he is an AGW Kool-Aid drinker...run, don't walk to the nearest exit.

I have found that the Orajel labeled "Night Time" works much much better than the regular Orajel. It is thicker and stays in place.
I don't have a dentist that does emergency stuff...I go to a dental clinic *sigh* but they have made me not so scared of dentists. You get scarred for life when you go to the dentist for the first time as a kid and he fills 9 cavities with no Novocaine

Thanks for the awesome advice, and +10 on the 2A supporter!!

I was (about 2 lifetimes ago) a dental assistant in the military, so I have at least enough knowledge to be dangerous, but I do have a question, tho...

Say a SHTF situation occurs, someone is in the middle of BFEgypt, no dentist or MD (or even someone who was an assistant or even a hygienist) for miles... and one gets an abscess?

I know this is dire straits last ditch info, but what would be a safe course of action for someone to take? I've heard of a book "When there is no dentist" - is it worth having, and would something like that help out in a "situation", or just dig someone into a deeper hole?

We are all about the worst case "what if" scenarios here.

In this situation it will be all about the infection!

Use whatever you have to treat infection:

Oil of oregano, tea tree oil, colloidal silver, fish antibiotics, fill in here with your favorite herb/root/berry/tree bark that fights infection.

Some of the herbal treatments could be inadequate to worthless or even make you sicker.

I recommend that you familiarize yourself with medically effective herbs NOW, so that you will always have some skills available in these worst case scenarios.

Even if it is nothing more than rinsing with warm salt water, or placing a regular old tea bag on the gums to draw out the infection, there is ALWAYS SOMETHING that a wise prepper can do in worst case situations.

And, yes, I would recommend "Where there is no Dentist".

Some of its contents are pretty much BS, so read it through and ask me or your dentist about what it is saying and how useful the info would be in an emergency.

I have had many toothaches. The best nonprescription relief for pain that I have found is to get the maximum strength Orajel and a small travel size bottle of Listerine. Mix the two together and shake vigorously until the Orajel dissolves into the Listerine. Using approximately a cap full in your mouth, place the liquid on the side of the toothache and hold there for a few minutes gently swish it back and forth a few times. It kind of burns like a son of a gun at first but soon the pain subsides. For me this is been one of my best friends during toothaches when I have not been able to get to a dentist or Medical. This is by no means a cure, but it for me has helped tremendously especially holidays weekends or other times I couldn't get medical help. I've tried just Orajel itself and I've tried just Listerine itself but for some reason the combination of the two seems to be able to get into places or one or the other cannot get by itself. Take this for what it's worth but I keep both of these items in my bug out bag.


I hope this information helps somebody else as much as it helped me.

Fish Zole (Metronidazole) 250 mg would be moderately useful.

Fish Mycin (Erythromycin) 250 mg would be much more beneficial. A little pricey, but pretty effective. Normal human dose is 250 mg three times per day for ten days.

Thanks Doc for your informative thread. Myself, I haven't been to a dentist in almost 40 years, and will never go to one ever again because of what a dentist did to my teeth when I was a youngster. He said I had cavities and charged my parents for silver fillings but put in "temps" for at least four or five years, every year charging for drilling his temps out and putting in new ones. By the time we went to a different dentist, my teeth were drilled out beyond repair.

I have since done what I can, on my own, and have found that swishing with water with baking soda and salt takes care of almost any issue.

Yes, I have used salicylic acid in a few cases, and I have found that ora-gel is a great thing to have around.

I have found what works for me, as everyone should do for themselves.

There are some horrible dentists out there for sure. You wouldn't want to know what I've seen. Thank God that they are pretty rare.

You have indeed found things that work. I often recommend brushing with baking soda, and when the gums bleed frequently, I recommend brushing with a paste made daily of baking soda and peroxide.

Rinsing with warm salt water is good for lots of oral issues including sore throats.

Yes, bentonite mixed with oil of clove would help with toothache and could possibly harden enough to use as a temporary filling.

BTW, the guy behind the whole 'root canals are bad' thing was Dr. Hal Huggins, the single biggest quack in recent dental history.

And, if you could truly prevent cavities by using herbal medicines, you and I could make millions perhaps billions. Please share and I'll give you the credit.

I hope you won't mind if I research them thoroughly before we go to market?

This book saved my life!

Have you tried the tips in the book; The Complete Guide to Natural Toothache Remedies and Re-mineralization. It is a great resource for just about any tooth related issue. In many cases you dont even need to see a dentist, just a few short term lifestyle changes and proper nutrition are all that is mecessary.

A few years ago I had an infected tuth that started giving me problems on a Friday. Over the weekend it turned into the worst pain of my life. I've had three kidney stones and broken bones and the tooth was worse. The pain was so bad I couldn't keep the narcotics down.

Since it was the weekend nobody could get me in to help me. By Monday when I had the root canal it was night and day and I felt like an amazing weight had been lifted off of me.

There is nothing like a toothache. I can see why somebody would go crazy and take an ice skate to knock the tooth out. I was literally contemplating grabbing a wrench and taking it out myself that bad.

I have often treated the indigent/financially challenged. I consider it a privilege to take another human being out of pain or life threatening infection. After all, how often do you get an opportunity to make that much difference in someone's life?

Any infection can kill you especially post SHTF.

Dental infection is especially dangerous to spread to the heart and brain. Seriously bad pizza.

Stock up on antibiotics NOW! Read up on fish antibiotics.
